Employee Health Check | COVID-19 Resources

Fill out the Health Check Screening below if you have any COVID-19 symptoms. The following guidelines will be adhered to by all Instructors and Staff. These guidelines are in alignment with Cal OSHA EST guidelines issued on June 17, 2021.  

1. Stay Home if you have any COVID-19 symptoms
2. Inform your manager
3. Fill out the Health Check, which triggers campus response, including Mt. SAC Contact Tracing
4. Do not come to campus until you have been cleared by a medical provider or Mt. SAC Contact Tracing
