Enrollment Verification
Enrollment verifications can be requested at the Admissions and Records Office by
the student with photo identification. You may also complete and submit the request
online, go to the Admissions Forms webpage, and locate the Enrollment Verification/Proof of Non-Enrollment form. Complete the form and follow the instructions on the webpage on how to submit
the form.
An enrollment verification will include the following information:
- Current and previous enrollment (Dates of Attendance, Full-Time or Part-Time Status)
- Attempted Hours and GPA
- Cumulative Hours and GPA
- Degree or Certificate Awarded
- Student Schedule
- Enrollment History
- Proof of Non-Enrollment
Students who need to verify enrollment for Unemployment Benefits purposes (EDD) must contact Renu Katoch, Adult Basic Education, at (909) 274-4845 (located in Building 30).
Request for enrollment verification can also be made through National Student Clearinghouse. It will include the following information:
- Current Enrollment
- Dates Attended
- Degree or Certificate Awarded
- Declared Major
As per Family Educational Rights and Privacy Act of 1974 (FERPA), directory information can be released to a third-party without the student's
consent. Directory information includes:

- Major field of study
- Dates of attendance
- Degrees, honors, and awards received
- Grade level

The most recent educational agency or institution attended
